 20/6/22  Hi, I’m A Naga pravallika 3rd Sem medical student.This is an online e-log book to discuss our patient health data.This also reflects my patient centered online learning portfolio  An 40 year old women,housewife,resident of kammareddygudem,was admitted to the hospital on18/6/22(Saturday). CHEIF COMPLAINTS: Pain on the left side of the head,neck,arm,limb. Low urine output  Burning micturition 2years back she had pain on the left side of her head,arm,limb.she visited a private hospital they gave her medication.she use them for 2-3 months but still the pain is not subsided so,she discontinued her medications . She had a pain at loin since, 2years.USG showed partially distended urinary bladder and irregular bladder wall thickening .CUE showed 3-4 pus cells. She  was diabetic since 2years.she had pain in the ear since 1year and the pus comes out every 2-3days no pain but she had ictching when the pus comes out. PERSONAL HISTORY: Appetite-normal Diet-mixed Bowels-regular  Allergy-ye

An 27 year old man student,resident of suryapet came  to hospital for dialysis. 3years ago he had frequent vomitings upto 45 days  and went to hospital and treated with medication.From then he started going for dialysis. He had a difficulty in breathing and has hypertension.His heamohlobin levels was very low (7.6). Since 2days he was having a stomach pain and no appetite. Personal History  Diet-vegetarian Appetite- normal  Bowels-regular  Addictions-no  Family History  Hypertension-yes  Diabetes-no  Heart disease-no  Stroke -no  Asthma-no SYSTEMIC EXAMINATION  CVS thrills-no  Cardiac sounds -s1,s2,+ Cardiac murmurs-no RESPIRATORY  Dyspnoea-no Wheez -no Position of trachea-central Breath sounds -vesicular CNS conscious  Speech-normal Neck stiffness-no General Examinatio

 Hi, I’m A Naga pravallika 3rd Sem medical student.Thjs is an online e-log book to discuss our patient health data.This also reflects my patient centered online learning portfolio  An 47 year old women,tailor by occupation,resident of Nalgonda was admitted in the hospital on 2/6/22(Tue) with complaints of fever , body pains , loss of appetite. 10 years back she developed joint pains she was treated at private hospital asymptomatic until 8 months back.post covid vaccine she was treated with medication.patient had a history of decreed vision at age of 15 years.  Patient history Diet-mixed Appetite-decresed Sleep-disturbed General Examination  Conscious Coherent Pallor No icterus No clubbing of fingers No lymphadenopathy  No edema  A rash was present on the face
